WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a Nurse Practitioner with Palliative Care experience for an Inpatient and Outpatient position to join our team at Franciscan Health Munster to meet the growing demands for Hospice and Palliative Care services, which includes outreach to our nearby Dyer campus.

WHAT WE NEED:

Ideal candidate will have past Palliative or Home Health Care experience, with a focus on clinical excellence, and patient satisfaction. Candidate will display exceptional interpersonal skills and desire to develop strong ties with the community and existing medical staff. Provider will need to work collaboratively with physician to coordinate and deliver the highest quality of patient care.

WHAT YOU CAN EXPECT:

• Schedule: Time will be split between clinic, and inpatient rounding
• Clinic schedule in Munster: Monday and Thursday, 8:00 a.m. - 4:30 p.m.
• Rotating weekend call for inpatient only
• Average outpatient volume: 12 patients per week
• Average inpatient volume: 6 patients per day
• Telehealth offered

Qualifications:

• Master's Degree Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association Required
