Under limited direction, increase patient access to quality health care by providing nursing expertise in diagnosis and treatment to prevent, remediate or alleviate illness and promote health. Consults and collaborates with all members of the healthcare team, and provides training on geriatric care to health care providers.

Job Duties:
- Provides patient care, in the assisted living, nursing home, and ambulatory care settings.Â
- Conducts physical exams, complete health history summary, document patients progress, assist with patient care management.Â
- Provides training and geriatrics education to heath care providers.
- Conducts geriatric screenings and assessments.
- Provides training and education opportunities to community physicians, interdisciplinary teams and their students on rapid geriatric assessment.
- Supports the attending physician and medical director in medical direction activities such as monthly QAPI meetings, interdiscipinary care meetings, and inservice trainings.
- Documents clinical visits in paper and electronic patient charts and submits billing for visits.Â
- Performs other duties as assigned.
Minimum Qualifications:
Masters degree in nursing, Missouri RN license, and certification as an Advanced Practice Nurse required. Geriatrics and post-acute care experience preferred.Â

SSM Health and Saint Louis University have reached an âœagreement in principleâ to fully integrate SLUCare, the School of Medicines faculty clinical practice, with SSM Health. The proposed SLUCare-SSM integration is the result of a partnership that has grown between the School of Medicine, SLUCare, SSM Health. SLUCares integration with SSM Health would create one of the regions leading integrated healthcare networks with a new Academic Medical Group (SLUCare) and a Community Medical Group.
